Google 2-Step Verification - Log In Survey
Next button was not present.
Back button not available.
Special note: After it had taken a few minutes to take the screen shots the message appeared asking if I hadn't received the code yet and offered to resend it.

Walmart survey - same answers, really?
After completing a survey for Walmart❐ with all 10's I clicked the Continue button. The result was a browser level warning with the message,
"The page at https://survey.walmart.com says: You responded the same several times. Please consider your answers carefully."
Honorable mention for the options appearing as the Walmart logo. There must be some subconscious message here, regardless of my position of being Extremely Dissatisfied or Extremely Satisfied, they want to reinforce that the Walmart brand should be top of mind.

Coca-cola smart dispenser - fail
"Please wait while we load this screen" (that message displayed while I waited in line to order, waited for the food, and needed to get my drink.
Maybe dispense water in the mean time.

Windows 7 System Clock - Daylight Savings message
A few days before daylight savings time the system clock provided a notification that the clock is set to go forward 1 hour.
For comparison, this is what the system clock normally looks like.
